First Visit

Your initial appointment will consist of an examination, x-rays, cleaning, oral cancer screening, intra oral, fluoride varnish treatment and take home whitening trays, if applicable. 

Please assist us by providing the following information at the time of your consultation:

• A list of medications you are presently taking. (You will receive electronic forms to complete before your visit)
• If you have dental insurance, please make sure you complete ALL the electronic forms before your appointment. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Unless we have a consent form and the child is being seen at their school or community center.

Please update our office if you have any medical conditions that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.). This information should be given to us when you fill out your health history form.
